New Combat Actions for the Creatures in Tyranny of Dragons

The bugbear chief nurses a gushing wound in her side before issuing an order of retreat to her allies. A bright light forms around the angel’s dying body, blinding everyone near it as it passes on from the world of the living. With blood streaming down its torso, an enraged hill giant slams its fists into the ground and causes the earth around it to quake. 

Bloodied & Bruised is a series providing new combat actions for the creatures in the 5e roster. When pushed to their limits in combat, these monsters, critters, fiends, and foes gain new abilities that aid them in battle, or perhaps spell their doom depending on the circumstances. Bloodied & Bruised takes inspiration from the "bloodied" condition from 4e, making entirely new combat actions that can completely change the nature of a fight.

In addition, Bloodied & Bruised also introduces several "death throes" to the game, marking the demise of certain creatures with a grand explosion, heavy fall, or other unique effect to make the slaying of a great monster a truly unique experience.

Examples of Bloodied Actions

Tiamat
When bloodied at 307 HP, Tiamat has the following features.

• Ability Recharge: Legendary Actions. When first bloodied, Tiamat regains all expended uses of her legendary actions.
• New Ability: Fling. While bloodied, Tiamat can use an additional charge of her legendary actions to make a Fling attack, but only if she just successfully hit a Large or smaller creature with a Bite attack on the same turn. Then, the creature Tiamat just bit is thrown up to 60 feet in a direction of Tiamat’s choice, landing prone. If a thrown target strikes a solid surface, the target takes 1d6 bludgeoning damage for every 10 feet it was thrown. If the target is thrown at another creature, that creature must succeed on a DC 23 Dexterity saving throw or take the same damage and be knocked prone.
• New Ability: Heavy Step. While bloodied, if Tiamat moves at least 20 feet and doesn’t leave the ground, she can replace her tail attack with a Heavy Step attack. While doing so, she can enter the spaces of Large or smaller creatures. Whenever Tiamat enters a creature’s space, the creature must make a DC 24 Dexterity saving throw. On a failed save, the creature takes 35 (10d6) bludgeoning damage and is knocked prone. If the dragon ends its turn in that space and the creature failed the save, the creature is grappled (escape DC 18) and restrained. On a success, the creature takes no damage, isn’t knocked prone, and is pushed 5 feet out of Tiamat‘s space into an unoccupied space of the creature’s choice. A creature that cannot move or chooses not to move suffers the consequences of a failed saving throw.
• New Ability: Hide of the Dragon Queen. While bloodied, Tiamat’s corporal body oozes hellish blood. Putrid acid, fire, and lightning weeps off her body, and the area around Tiamat’s body is filled with greenish poison as the floor at her feet is covered in ice. Any creature who touches Tiamat or hits her with an unarmed strike takes 5 (1d10) acid damage and 5 (1d10) lightning damage. In addition, a creature that starts its turn within 5 feet of Tiamat takes 5 (1d10) poison damage and must succeed on a DC 24 saving throw or slip on the ice and fall prone. Tiamat is not affected by the ice.
• New Ability: Outsmarted, Outwitted, Outplayed. While bloodied, Tiamat has advantage on Intelligence, Wisdom, and Charisma saving throws.
• Trigger Ability: Cornered. When first bloodied, Tiamat can use a reaction to make a Claw attack against a creature within range.
